Sometimes things happen and trying to come up with extra money can be tough.  We understand and will do what we can to help.

If you have a hardship, please fill out the form below and send it through.  Please note, that sending this through, you are obligated to do the following:  

​

  1.  Bring your child to practices and games.

  2. Stay in contact with your coach.

  3. Dropping out before season ends, you will be required to pay the fee in full.  

  4. You will need to send in your form and then sign it in front of me, so it can be notorized.    This is due to people taking advantage of the league and not showing up for practices or          games.  Being notarized we will take charge in getting the payment and you will be                responsible for all costs for this.
